Total Reconstruction with Dental Implants
Total reconstruction with dental implants is often recommended when the patient has lost all their teeth or has serious dental problems that require the extraction of most, if not all, of the remaining teeth.
Thanks to technological advances and modern techniques in dental implantology, it is possible to reconstruct a new, solid and functional dentition using dental implants as support for prosthetics.
This complex procedure involves the strategic placement of dental implants in the upper and/or lower jaw, followed by the attachment of a complete or partial dental prosthesis on these implants. This allows the restoration of the ability to eat, speak, and smile confidently.
Total reconstruction with dental implants requires thorough planning and an accurate assessment of the patient's oral health. As each case is unique, a team of dental professionals will collaborate to design a personalized treatment plan that meets the patient's specific needs.
It is important to note that total reconstruction with dental implants is a surgical procedure and may require a healing period, regular visits to the dentist, and good oral hygiene to maintain the implants and prosthetics in good condition in the long term.
If you are considering a total reconstruction with dental implants, it is strongly recommended to consult a dentist specialized in dental implantology to assess your situation and determine if this procedure is right for your needs.
Double ALL ON 4
The ALL ON 4, ALL ON 6, and ALL ON 8 Solutions
The principle is to place between 4 and 8 implants (ALL ON 4, ALL ON 6, and ALL ON 8) to support the entire jaw. The goal is to be able to reconstruct a complete dentition that can range from 12 to 14 crowns. Total reconstructions are complete jaw reconstructions with fixed solutions (not removable by the patient).
ALL ON 4 or ALL ON 6, how to choose?
An example of an ALL ON 4 result
The choice between the ALL ON 4 or ALL ON 6 implant solution (or 8) is often determined by the amount of bone available to place the implants.
The implants are strategically placed to bear the mastication load transmitted by the crowns.
The crowns are of course linked to each other (implant-supported bridge) to distribute the load and benefit from a dentition with more teeth than support points (there are more teeth than implants).
With the ALL ON 4, the bridge is often larger and covers some of the gum. We then speak of a complete prosthesis on implants.
The ALL ON 6
Implant-supported bridges on ALL ON 6
With the ALL ON 6, it is often just crowns; there is rarely any gum coverage.
ALL-ON procedures refer to operations of rehabilitation, or complete reconstruction of the dentition of edentulous patients or patients with unrecoverable teeth by other techniques of modern dentistry. When using these techniques, none of the old teeth or roots are used.
How long to complete a global jaw reconstruction ?
Global reconstruction are done in 2 steps:
STEP 1 - Implants surgery. Implants are inserted in the bone. This steps takes usually 1 or 2 days.
Waiting for bone healing from 4 to 5 months.
STEP 2 - Crowns are screwed on Implants - This step usually takes 5 to 7 nights and 2 to 3 visits to the clinic during that time.
No healing period to be expected after the crowns.
Good dental hygiene is recommended after the installation of an ALL ON solution.

About
4 Implants and 12 Teeth per Jaw
All on 4 on Upper and Lower
The ALL ON 4 procedure is the complete rehabilitation procedure that uses the fewest implants per jaw. As the name suggests, 4 implants are placed in the jawbone to support a complete prosthesis that is usually screw-retained in the implants. Screw-retained prostheses are prostheses attached to the implants by screws instead of being cemented on implant abutments.
Unlike ALL ON 6 procedures, the prosthesis is a complete prosthesis that goes above the gum. It is not just crowns.
Screw-retained prostheses as well as the ALL ON 4 solution are preferred when very little bone remains, especially on the second premolars and molars, where additional dental implants for ALL ON 6 solutions are usually placed.
Therefore, this procedure is preferred for the following reasons:
- Lack of bone to place more endosseous implants.
- Avoiding bone grafts.
- Financial motivation, as the ALL ON 4 solution is less expensive than other solutions.
Advantages of All-on-4
- Complete jaw restoration in a single procedure.
- Quick recovery compared to other dental techniques.
- No need for bone grafts.
- Improvement in facial aesthetics.
Disadvantages of All-on-4
- The prosthesis is often larger.
- The prosthesis covers some of the gums in most cases.
- Minor adjustments may be necessary over time.
How Long Does All on 4 Take?
2 trips will be required to complete the treatment due to the waiting time for osseointegration.
- The first trip is for 3 working days.*
- The second trip is for 5 working days.*
A period of 4 to 6 months is required between the trips.
*The exact timing of each trip and the wait between them should be discussed with the oral surgeon.
An ALL ON 4 is completed in a total of 6 months.
ALL ON 4 Implant Brands
Dental implants for ALL ON 4 are often specifically engineered for ALL ON 4. They are often different from standard implants. World-renowned brands such as Nobel Biocare, Alpha Bio, Bego, Straumann, Dio, and many others are most commonly used. These brands are known for their quality and durability, ensuring long-term results.

About
6 Implants and 12 Teeth per Jaw
All on 6 on the upper jaw
The ALL ON 6 procedure is a complete rehabilitation procedure that uses more implants per jaw and can support a complete bridge of ceramic-metal and Zirconia dental crowns. As the name suggests, 6 or 8 implants are placed in the jawbone, with implants typically placed every 2 crowns. Crowns can be placed in a transvissed or cemented mode depending on the implant brands.
This solution is a procedure that allows patients to regain their jaw, almost as it was before. The chewing sensation is almost back, and strength is very important due to the load distribution every 2 crowns.
12 crowns are ideally placed, and 14 crowns are placed for the ALL ON 8 solution.
To place these implants, good bone quality is necessary, and if there is not enough bone, bone grafting and often sinus floor elevation (sinus lift) may be necessary before implantation.
Therefore, this procedure is preferred for the following reasons:
- Patients desiring complete and nearly perfect rehabilitation.
- Patients who do not want appliances and want to regain perfect crowns.
- To avoid bone loss by fixing the implants and if necessary performing sinus lifts and/or bone grafting.
- 100% fixed solution.
- Patients regain their jaw.
- Patients wanting Zirconia or Emax teeth. Zirconia and Emax crowns can be placed on implants and complete the ALL ON 6 or 8 solution.
Advantages of All-on-6
- Complete jaw restoration in a single procedure.
- Zirconia or ceramic-metal crowns.
- Improved facial aesthetics.
Disadvantages of All-on-6
- Adequate bone is required.
- Crowns are connected in a bridge.
How Long Does All on 6 Take?
2 trips will be required to complete the treatment due to the waiting time for osseointegration. If bone grafting is necessary, it is likely that 3 trips will be required.
- The trip for implantation and/or bone grafting is 3 working days. Generally, a few months are needed after bone grafting before placing the implants.*
- The second trip is 7 working days.*
*The exact timing of each trip and the wait between them should be discussed with the oral surgeon.
** Zirconia crowns can also be placed.
